RemoteFocus will provide an easy-to-use form where you can send us referrals, and we will check eligibility and conduct an initial assessment with the patient.
We will form a patient-centric care plan for your patients, and ensure that you are the primary care doctor that the patient will see if they need an in-person visit.
Similar to our homecare agency partnership setup, you as the physician will have access to a read-only portal for any and all information gathered by RemoteFocus case managers and clinicians.
RemoteFocus will communicate with you as the primary care physician should any patient interventions or alerts occur.
