Virtual Urgent Care and Case Coordination

A 78-year-old female was referred to RemoteFocus for virtual care services. The patient suffers from Parkinson’s, Anxiety/Depression, Polyneuropathy, and Gait abnormalities.

Following a telehealth visit, the patient consented to chronic care management services.  She received support in obtaining necessary Durable Medical Equipment (DME) such as a Standard Transport Chair and tripod cane, delivered within 48 hours of the initial visit, addressing urgent needs.

Upon completion of the allotted hours with , the patient
was denied home care services due to Medicaid limitations. Although the case manager offered guidance on obtaining Medicaid, the patient declined assistance.  Nevertheless, the case manager successfully arranged alternative home care services for the patient.
