DME Allocation and Avoiding Falls

An 83-year-old male suffers from Parkinson’s, Hypertension, and Gait abnormalities. The family faced challenges securing essential Durable Medical Equipment (DME) for the patient.

The patient experienced a fall from his bed resulting in hospitalization. Following discharge from rehab, the RemoteFocus clinician and case manager facilitated the provision of essential DME supplies, including a semi-electric hospital bed with bed rails, a mattress, a tripod cane, and a rollator.

Subsequently, the patient has remained fall-free and avoided hospital readmissions.
